In addition to the critically outspoken Gus Grissom, the critically outspoken safety manager Thomas Baron was killed in an accident.

Thomas Baron, wife and step-daughter killed in a crash with a train.
Thomas had just testified before congress that the Apollo program was in bad shape.
His 500 page report disappeared.
